How to Request a Certificate of Graduation

If you wish to request a certificate after graduation (or completion of your program), please visit the Certificate Office at your respective campus in person or submit your request by mail.
Please note that we do not accept requests by phone or email.

About Issuance Fees

The university charges a fee for issuing certificates for graduates and program completers.

Eligible for fees

Graduates, program completers, students who have withdrawn, and students who have been dismissed (Except when the application is submitted in the fiscal year in which the date of graduation, completion, withdrawal, or expulsion falls.)
*Includes non-degree students such as auditors and research students.

Issuance fee: (per copy)

Certificate in Japanese 300 yen
*Certificates of credit completion for the Japanese Language Teacher Training Program are issued in Japanese.

Certificate in English 500 yen

Payment Methods

(1) For requests submitted by mail

*Please make your payment at a Japan Post Bank (post office) counter using the designated form provided by the post office. (Please do not use Japan Post Direct or similar services.)
*Please be sure to write "Certificate" in the "Remarks" section of the bank transfer form.
*Any fees associated with the bank transfer are the responsibility of the applicant.
*Please be careful to ensure there are no overpayments or underpayments.

(2) When submitting a claim in person 

After applying at the certificate issuance office on each campus, please pay in cash at the accounting office.
*Please bring the exact amount to avoid receiving change, and visit during weekdays from 8:30 AM to 12:00 PM or 1:00 PM to 5:00 PM. (Please note that transactions cannot be processed between 12:00 PM and 1:00 PM due to the lunch break.)

Regarding the processing time for issuance

For general certificates such as "Graduation (Completion) Certificates" and "Transcripts," the processing time from receipt to shipment is approximately 2 to 3 business days (excluding weekends and holidays).
Please note that "Certificates in English," "Academic Ability Certificates," and "Certificates of Course Completion" for qualifications may take approximately one week or longer to process after receipt, depending on the content of the certificate.

Please note that the type and format of the “Certificate of Academic Achievement” may vary depending on your course credits and teaching experience; therefore, please be sure to contact the relevant office in advance.
Please note that processing may take longer if your request falls during the year-end and New Year holidays or summer break, or if the required documents are incomplete (e.g., missing information or inaccuracies) or if we are unable to confirm receipt of payment.

*For privacy protection purposes, we require you to present or submit identification documents for identity verification. We apologize for any inconvenience this may cause and appreciate your cooperation.
*The personal information you provide for identity verification will be used solely for the purpose of issuing certificates and will not be used for any other purpose.

If you are visiting the campus in person

Please send the following items—(1) “Certificate Issuance Request,” (2) a self-addressed stamped envelope, (3) identification documents, and (4) the “Direct Deposit Request Form and Receipt”—to the Certificate Issuance Office listed below.
*Please be sure to write “Certificate Issuance Request Enclosed” in red on the front of the envelope and clearly indicate your name and address on the back.

Alternatively, please fill in the following information on a blank sheet of paper or stationery.

  1. Date of application, full name, and phonetic spelling (if your name has changed since enrollment, please provide both names)
    *If submitting an English-language certificate, please also include the Romanized spelling.

  9. Purpose of use or recipient (e.g., to be submitted to the XX Certification Association for the purpose of obtaining certification, etc.)
Important Notes

(2) Return envelope

Envelope size: Long No. 3 or Square No. 2
*Certificates are A4 size. For sealed certificates, fold the document into quarters, place it in the Kochi University Certificate Envelope (Sealed Envelope), and seal it.
Please clearly indicate the recipient’s information (postal code, address, and name) and affix the required postage.
*Please address the envelope to the applicant or the authorized representative (agent) listed on the power of attorney.

The university will not send it directly to the recipient.

Postage rates vary depending on the number of certificates and sealed envelopes, as well as the size and weight of the return envelope.
Below are approximate weights for certificates and envelopes. Please calculate the postage based on the total weight and affix stamps to the return envelope.

(3) A copy of a document verifying your identity (such as a driver’s license or passport)

This document is required to verify that the application is being submitted voluntarily by the individual concerned, in accordance with personal information protection regulations.
Please note that this information will not be used for any purpose other than issuing certificates.
*Please note that blurry or incomplete images will be considered invalid.
*Please submit a copy of the document that does not include your My Number.With the introduction of the My Number system, some official documents are now issued with an individual number (My Number) included. When submitting documents for identity verification, if your My Number is listed, please cover that section before submitting a copy.

(4) "Direct Deposit Request Form and Receipt" (copies accepted)

Please ensure that the amount paid for the issuance fee is exactly correct.At a Japan Post Bank (Post Office) counterPlease use the form provided by the post office to make your payment. (Any transfer fees incurred when depositing funds into the designated account will be the responsibility of the applicant.) 
*Please note that we cannot issue the certificate if the payment does not cover the issuance fee or if we are unable to confirm receipt of the payment. Additionally, please be aware that we cannot refund any amount exceeding the required fee.
Please note that we will not return the "Transfer Payment Request Form and Receipt"; if you need the original, please attach a copy.

(5) Other Important Notes

1) A fee is required for the issuance of certificates for all former members, butFor those who graduated or completed their program within the current academic year, there is no fee for issuing certificates.
2) If you reside overseas, you must submit your application through a representative in Japan. If you do not have a representative in Japan, please contact the address and department listed below.
3) Request for a certificate by an agent

A power of attorney and copies of the identification documents listed in (3) above for both the applicant and the representative are required.
While there is no specific format for the power of attorney, please handwrite the following information:
・Date the Power of Attorney was created
・Agent’s address, phone number, name, and date of birth
・Scope of authorization (e.g., “I hereby authorize the above-named person to apply for and receive one English-language graduation certificate on my behalf”)
・Applicant’s address, contact information, name, seal, and date of birth

4) Issuance of Certificates After a Name Change
As a general rule, we do not issue certificates under a name that has been changed. Certificates are issued under the name used at the time of graduation.
If you require a certificate issued under your new name, please contact the following office or the designated department by phone in advance before requesting the certificate.

5) Regarding Teaching Licenses
For inquiries regarding the issuance of certificates of teaching license conferral, as well as the amendment or reissuance of teaching licenses, please contact the Board of Education that issued your license.
Please note that if you received your teaching license from this university upon graduation, please contact the Kochi Prefectural Board of Education, which serves as the central application office for university-issued teaching licenses.

6) If you wish to obtain a "Certificate of Qualifications Required for Appointment as a Social Welfare Officer," please request a "Certificate of Graduation" and a "Transcript."
